Long term Care and Support

Family based care and support. A person lives with the care providers and shares the same experiences of any normal family environment.

Shared Care
Line break

Shared care is where the care is shared, usually between a persons parents/long term carer and the shared lives carer. This is normally :-

  • to prepare a person for a long-term placement.
  • to enable a person to develop skills so that they can live independently.
  • to enable a person to adapt to life away from their parents to adapt to life away from their parents or family home, or
  • to provide an opportunity for their parents/ long term carers to have longer breaks.

Extended family support offered to people living independently in their own home, or, in accommodation provided by and Shared Lives Carer

Short breaks

Where a person stays with a shared lives carer for a holiday; weekend or overnight day stay. This may be a planned arrangement or during an emergency.

Independent living Services
Line break

This service is aimed at those people that need and want to move towards living independently. No personal care is provided. The person would usually live in the same household as their landlord, who would provide them with low-level housing related support for no more than 5 hours per week. The service supports people to develop new and maintain existing skills, which will hopefully enhance their confidence, to live more independently.

By tailoring packages of support to an individuals needs and aspirations we aim to be

PERSON CENTRED and OUTCOME FOCUSSED.

Our support providers offer a variety of skills and experience that can be based in their own homes or in the community. Opportunities could include:

  • Volunteering
  • Re-cycling:
  • Travel training.
  • Lifeskills.
  • Art and craft
  • Theatre.
  • Gardening
  • Community projects.

Home from Hospital Service

This provides support for service users when they are discharged from hospital or accommodation until they regain enough confidence to go home.

Shared lives can be part of the circle of support for people living in their own homes

We can support individuals to:

  • Increase or retain levels of independence.
  • Access the community.
  • Budget.
  • Learn lifeskills.
  • Enjoy community facilities.

We will support people with what they need at the time they need it.
